Change name or address

If you’re a Designated Premises Supervisor and have changed your name or address, you’ll need to let us know so we can update the details on your licence. This costs £10.50.

Use our online form to let us know about your change of details. It takes about 5 minutes to complete.

At the end of the form we’ll ask you to pay by credit or debit card so please have your card details to hand.

If you are the Designated Premises Supervisor, you must inform us of any changes to your name or address. Failure to notify us of a change in name or address is committing an offence under Section 33(6) of the Licensing Act 2003. A person guilty of such an offence is liable on summary conviction to a fine not exceeding level 2 on the standard scale, for example, £500.
