Healthy Homes
Our Healthy Homes service is in place to ensure people are living in healthy and safe homes, free from significant hazards to avoid hospitalisation.
Our Goal
- To ensure our residents are living in safe and healthy homes
- To avoid hospital admission from preventable hazards within the home
- To provide a holistic approach to each assessment, checking all amenities and utilities are in place
- To seek additional support for clients from external agencies where appropriate
Funding
The service is funded by the Better Care Fund to achieve:
- Healthy living
- Supported lives
- Improved wellbeing
- Safeguarding residents
- Quality living for our residents
Eligibility
To be eligible clients must:
- Be referred by a professional
- Have a long-term health condition exacerbated by their living conditions
- Be living in a property where clutter is preventing essential minor repairs, impacting their health and safety
- Be living where clutter is putting the client at risk of hospitalisation
Referrals
The service is tenure neutral, this means that whether you are a homeowner, social housing tenant, private or council tenant the service is available to you.
Referrals are to be made by professionals. Self-referrals will not be entertained at this time.
Only online referrals through the referral form link will be accepted.
